hospital is a priority of OSF HealthCare.<\/p>\n<p>By ensuring patients are treated in a timely fashion with primary care and proper outpatient management, OSF works to reduce or avoid the need for them to be hospitalized for care.<\/p>\n<p>In fact, physicians state that reduced hospital admissions are within reach because several chronic medical conditions typically respond well to outpatient care.<\/p>\n<h2>Chronic conditions that need routine monitoring<\/h2>\n<p><strong>Those conditions include:<\/strong><\/p>\n<div class=\"columns\">\n<div class=\"col2\">\n<ul>\n<li><a href=\"https:\/\/healthlibrary.osfhealthcare.org\/Search\/85,P01497\">Urinary tract infections (UTIs)<\/a><\/li>\n<\/ul>\n<\/div>\n<\/div>\n<p>\u201cAs we shift from a volume-driven to a value-driven care model, caring for patients in the ambulatory (outpatient) setting puts greater emphasis on health and wellness, as well as reducing the total cost of care,\u201d said <a href=\"https:\/\/www.osfhealthcare.org\/providers\/profile\/460\/mark-a-meeker-do\/\">Mark Meeker, DO<\/a>, vice president of community medicine for OSF HealthCare. \u201cFocusing on outpatient care also allows us to reserve inpatient care beds for patients who need them most.\u201d<\/p>\n<h2>Why does this matter to patients?<\/h2>\n<p>Patients with chronic health issues need ongoing care management, said <a href=\"https:\/\/providers.osfhealthcare.org\/provider\/Brian+S.+Curtis\/1465391?unified=Curtis&amp;sort=networks%2C%20relevance\">Brian Curtis, MD<\/a>, vice president of specialty medicine for OSF HealthCare.<\/p>\n<p>\u201cWe want to provide high quality care to the patients we serve not just when they are sick \u2013 but to keep them well,\u201d Dr. Curtis said. \u201cBy providing the right care in the right place at the right time, we can help them live healthy and full lives and avoid costly hospitalization for those conditions that can be easily managed.\u201d<\/p>\n<h2>How to achieve this<\/h2>\n<p>Dr. Meeker said engaging with your primary care provider early can prevent hospitalizations.<\/p>\n<p>\u201cBy tracking these preventable hospitalizations, OSF HealthCare can assess the quality of health services in the communities we serve,\u201d he said. \u201cOur team-based approach to care encourages patients to take a more active role in managing their own health.\u201d<\/p>\n<p>For example, a patient with congestive heart failure may be hospitalized for complications if their condition is not appropriately monitored or if they do not receive the appropriate education or self-management.<\/p>\n<h2>Seek care right away<\/h2>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-10693 alignright\" href=\"https:\/\/www.osfhealthcare.org\/providers\/search\/?near=&amp;max_distance=&amp;specialty=6&amp;specialty=3&amp;specialty=21&amp;facility=&amp;is_preferred=on&amp;gender=&amp;language=\">primary care provider<\/a> when they notice symptoms changing or worsening.<\/p>\n<p>Dr. Curtis agreed.<\/p>\n<p>\u201cPatients need to understand we are there for them. It\u2019s not a bother to have them come in to discuss their condition \u2013 those steps may keep them out of the hospital down the road,\u201d he said. \u201cDon\u2019t put it
