But when it comes to performing brain surgery, reducing your margin of error by even the tiniest fraction can have huge implications.<\/p>\n<p>\u201cDeep brain stimulation should always be performed with the best accuracy possible. By modifying them electrically, we can modify the function related to those areas. We are able to reverse those symptoms almost completely, especially when treating movement disorders,\u201d Dr. Maldonado said.<\/p>\n<p>\u201cWith epilepsy, there is a very tiny nucleus in the brain called the anterior nucleus of the thalamus. It\u2019s only a few millimeters long. <a href=\"http:\/\/healthlibrary.osfhealthcare.org\/Search\/135,38\">Deep brain stimulation<\/a> might be able to dramatically improve the severity of seizures, and in some cases, even stop them.\u201d<\/p>\n<h2>A more precise placement<\/h2>\n<p>When targeting such small structures of the brain, precision is key. Dr. Maldonado\u2019s approach to this procedure is unique. By using a robotic assistant, he\u2019s able to be extremely precise in delivering this life-changing treatment.<\/p>\n<p>With this approach, <a href=\"https:\/\/www.osfhealthcare.org\/services\/neurosciences\/\">OSF HealthCare Illinois Neurological Institute<\/a> is among only a handful of centers in the U.S. using the ROSA robotic surgical assistant to perform deep brain stimulation.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-8946 alignright\" src=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/03\/neuro-rosa-revised-sd4v2-300x200.jpg\" alt=\"Dr, Maldonado performing deep brain stimulation\" width=\"300\" height=\"200\" srcset=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/03\/neuro-rosa-revised-sd4v2-300x200.jpg 300w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/03\/neuro-rosa-revised-sd4v2-500x333.jpg 500w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/03\/neuro-rosa-revised-sd4v2-400x267.jpg 400w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/03\/neuro-rosa-revised-sd4v2.jpg 600w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/>The ROSA robot helps calculate the exact position and trajectory needed to reach the targeted area of the brain. The neurosurgeon places a very thin wire with small electrodes on its tip that will deliver electrical stimulation to a small volume of tissue.<\/p>\n<p>A standard accuracy registration for deep brain stimulation usually falls between 0.8 and 1.2 millimeters.<\/p>\n<p>By using the ROSA robot, OSF INI has consistently demonstrated an accuracy within 0.2 and 0.4. On one recent surgery, the team <a href=\"https:\/\/www.linkedin.com\/posts\/andr%C3%A9s-maldonado-naranjo-86607ba5_dbs-seeg-epilepsysurgery-activity-6626572577357783040-k3VP\">registered an accuracy of 0.15 millimeters<\/a>, which is the second best in the nation according to ROSA data.<\/p>\n<p>\u201cThat\u2019s about the thickness of a hair,\u201d Dr. Maldonado said.<\/p>\n<p>Those fractions of millimeters can make a big difference in the effectiveness and safety of deep brain stimulation. If the electrode isn\u2019t implanted in just the right place, the neurosurgeon may have to penetrate the brain several times, which increases the risk of bleeding.<\/p>\n<p>But if the electrodes are implanted in just the right place \u2013 the effects are astounding while minimizing the risks.<\/p>\n<h2>A game-changer for epilepsy treatment<\/h2>\n<p>In people with uncontrolled epilepsy \u2013 <a href=\"http:\/\/healthlibrary.osfhealthcare.org\/Search\/40,EpilepsySeizuresQuiz\">seizures<\/a> that can\u2019t be controlled with medications or other treatments \u2013 surgical treatments are another option.<\/p>\n<p>The robot is used to insert tiny electrodes into the brain through a small opening in the skin and skull to target the areas doctors suspect are causing seizures. This electrical monitoring allows them to see exactly which part or parts of the brain are causing the seizures.<\/p>\n<p>Many times, this so-called \u201cepileptogenic zone\u201d can be surgically removed to either cure epilepsy altogether or significantly improve the seizures.<\/p>\n<p>In some cases, surgery isn\u2019t an option. In those instances, a neurosurgeon may use deep brain stimulation or implant a device called a responsive neurostimulator, or RNS, which works like a <a href=\"http:\/\/healthlibrary.osfhealthcare.org\/MultimediaRoom\/VideoLibrary\/?e=0#player:138,w1711\">pacemaker<\/a> in the brain to detect seizures and stimulate the brain to stop them.<\/p>\n<p>\u201cYou could have a child who is having 60 to 70 seizures a day. If you are able to discover the area of the brain that causes seizures, he could be completely seizure-free with surgical treatment.
