Everything sounds fine. You fit right in.<\/p>\n<p>But you don\u2019t.<\/p>\n<p>In reality, you\u2019re moving slow. When you talk, people next to you strain to hear. But you don\u2019t know.<\/p>\n<p>This is the dilemma of people with <a href=\"http:\/\/healthlibrary.osfhealthcare.org\/Search\/85,P00800\">Parkinson\u2019s disease<\/a>, where signals from your brain to your muscles grow progressively weaker. Your brain orders a big step or a loud, clear sentence, but your leg translates the step as a shuffle, your vocal cords respond with a whisper \u2013 and you have no idea.<\/p>\n<h2>Therapies help manage Parkinson\u2019s<\/h2>\n<p>There is no cure for Parkinson\u2019s. But thanks to therapies like LSVT BIG\u00ae and LSVT LOUD\u00ae, offered by <a href=\"https:\/\/osfhealthcare.org\/rehabilitation\">OSF Rehabilitation<\/a>, symptoms can be managed.<\/p>\n<p>\u201cIf we didn\u2019t have programs like those, I don\u2019t know what we would have done,\u201d Gladys Schmidt said.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-8886 alignright\" src=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd1-300x200.jpg\" alt=\"John Schmidt and his wife, Gladys\" width=\"300\" height=\"200\" srcset=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd1-300x200.jpg 300w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd1-500x333.jpg 500w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd1-400x267.jpg 400w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd1.jpg 600w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/>John, her husband of 52 years, has Parkinson\u2019s. Gladys first started to notice changes in his gait and speech a few years ago, but didn\u2019t give them too much thought because episodes were intermittent. Then their adult sons noted John\u2019s slurred speech when talking on the phone. To John, though, everything felt normal. Almost.<\/p>\n<p>\u201cI fell once or twice, but I just thought I was getting older, or something had tripped me that I didn\u2019t notice,\u201d John said. \u201cBut I knew something wasn\u2019t right.\u201d<\/p>\n<p>For a year, his doctor had been ordering tests, which John passed with flying colors. Then, while sitting in his hotel room on a business trip in October 2015, John couldn\u2019t decipher a single word of the notes he had taken that day.<\/p>\n<p>John\u2019s doctor sent him to a neurologist for more tests. A year later, he was finally diagnosed with Parkinson\u2019s.<\/p>\n<p>\u201cThat was actually a relief, knowing what it was,\u201d John said. \u201cI figured I could do something about it, now that I knew what was wrong.\u201d<\/p>\n<h2>Big movement, loud speech<\/h2>\n<p>Lee Silverman Voice Treatment (LSVT) BIG and LOUD are named for a woman with Parkinson\u2019s who inspired her therapist to do research that led to the program\u2019s founding in the 1980s. LSVT LOUD was the first evidence-based therapy that proved to make a long-term difference in the lives of Parkinson\u2019s patients. LSVT BIG soon followed.<\/p>\n<p>LSVT BIG addresses movement, while LSVT LOUD deals with <a href=\"https:\/\/www.osfhealthcare.org\/rehabilitation\/services\/speech-language-therapy\/\">speech<\/a>.<\/p>\n<p>\u201cThe programs are designed to help the person understand that in their head, they feel like they are moving and talking normal, but what the rest of us see and hear is not normal,\u201d said Brittany Heidemann, MS, CCC-SLP, a speech language pathologist at <a href=\"https:\/\/www.osfhealthcare.org\/locations?locationType=Rehabilitation%20Center\">OSF Rehabilitation in Peoria<\/a>.<\/p>\n<p>When a person with Parkinson\u2019s is referred to OSF Rehabilitation, they undergo an evaluation that includes audio-visual recordings of their words and actions.<\/p>\n<p>\u201cUsually at first, they\u2019re in denial,\u201d Brittany said. \u201cBut then we let them watch themselves, and they go, \u2018Oh, you\u2019re kidding me. That\u2019s what I look like? That\u2019s why people tell me they can\u2019t hear me?\u2019\u201d<\/p>\n<h2>Recalibrating normal<\/h2>\n<p>Therapists work with their patients to set personalized goals. Those goals vary, depending on the individual, and they may be as simple as being able to play on the floor with a grandchild. Other goals are more standardized, such as maintaining a speaking volume of 85-90 decibels.<\/p>\n<p>The approaches to therapy are consistent.<\/p>\n<p>\u201cWe push big and loud,\u201d said Mike Holloway, PT, DPT, a physical therapist at OSF Rehabilitation. \u201cIf you exaggerate and go bigger, you will end up at normal. We just have to recalibrate what sounds and feels normal to them.\u201d<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-8892 alignright\" src=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd2-300x200.jpg\" alt=\"elderly man listening intently\" width=\"300\" height=\"200\" srcset=\"https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd2-300x200.jpg 300w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd2-500x333.jpg 500w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd2-400x267.jpg 400w, https:\/\/osf-blog.live.imagescape.com\/blog\/wp-content\/uploads\/2020\/02\/big-loud-therapy-sd2.jpg 600w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/p>\n<p>The programs are intense, each requiring four one-hour sessions a week for four weeks.<\/p>\n<p>LSVT BIG starts with relatively simple activities, such as getting out of bed or walking up and down stairs. An occupational therapist will work with the patient\u2019s hands and arms, addressing fine motor skills that are useful in getting dressed or cooking a meal. A physical therapist will work to develop and maintain leg strength, balance and walking.<\/p>\n<p>An LSVT LOUD session includes a series of voice stretching exercises, each repeated 15 times, and working on everyday phrases like, \u201cIs the mail here yet?\u201d There are also practical application exercises, such as conversing with OSF Rehabilitation staff.<\/p>\n<p>An alternative program to LSVT LOUD is SPEAK OUT!\u00ae, which starts with individual therapy and advances to The LOUD Crowd\u00ae, a weekly group session. SPEAK OUT! also includes components that address memory issues and finding the right word to use, as Parkinson\u2019s also slows down that process.<\/p>\n<p>All of the programs are accompanied by 30 days of homework for the patients to do on their own.<\/p>\n<h2>Rehab program saves his speech<\/h2>\n<p>John Schmidt has been through all of the programs.<\/p>\n<p>\u201cI started with LOUD. It did feel like I was shouting, but I started talking better. I could tell,\u201d John said. \u201cThe problem is that once you finish the therapy, you\u2019re on your own to keep up with the exercises. I did not, and my voice again deteriorated.\u201d<\/p>\n<p>Brittany recommended the SPEAK OUT! Program, and John now participates in The LOUD Crowd.<\/p>\n<p>\u201cI really believe that I would not be communicating with my voice were it not for this program,\u201d John said.<\/p>\n<p>\u201cLater, I did the LSVT BIG. I really like the exercises. I have been engaged in a program called <a href=\"https:\/\/www.osfhealthcare.org\/blog\/parkinsons-disease-changed-my-life-but-i-am-fighting-back\/\">Rock Steady Boxing<\/a>, which is great. The strength that I had gained really has helped me. The BIG program helped me focus on exercises that helped my balance.
